To enroll in any of our obedience classes, please print off the enrollment form and completely fill it out.  If you are participating in Rally obedience, we ask that you only fill out the top two sections of the form and sign at the bottom.  Sections 2,3 and 4 can be left blank for Rally obedience dogs.  When you have the form completed, fax or bring it by our office along with a copy of your doggie's most recent vaccination records. 

The following vaccines are required:

• Bordetella
• Rabies
• Distemper/Parvo

If you prefer to enroll over the phone, please call us at (630) 969-7529.  We are in the office from 6:30am to 6:30pm Monday through Friday and would be happy to process your enrollment over the phone.    

Payment for the training classes is due at the time of enrollment.  This ensures your spot in the class and completes the enrollment.
